  1. How do I make a claim? Claims for medical expenses can be made by completing the Volunteer Accident Insurance: Non-Dental form in its entirety. This is a single form consisting of 3 pages.

          Page 1: The claimant's section is to be completed by the volunteer. The School

          Declaration is to be completed by the school principal.

          Page 2: Is to be completed by the volunteer.

          Page 3: The Physician's Statement section is to be completed by the attending physician,

          but does not need to be completed for claims $100 or less or for ambulance bill only.

          Claims for dental expenses can be made by completing the Volunteer Accident

          Insurance: Dental form in its entirety. This is a single form consisting of 3 pages.

        

          Page 1: The patient information section is to be completed by the volunteer. The School

          Declaration is to be completed by the school principal.

          Page 2: Is to be completed by the volunteer.

          Page 3: The attending dentist statement is to be completed by the dentist.

  2. Is there a Deadline for making a claim?Yes. Claims should be submitted to AXA Assurances Inc. within 30 days of the date of the accident for which the claim is being submitted.
  3. If I am unable to submit my clams forms before the 30 days deadline, what should I do? Contact AXA Assurances Inc. in writing, outlining your reasons for not meeting the deadline. Please note: No claim will be considered later than one year after the date of the accident.
  4. Whom should I contact if I have questions about my claim? Questions concerning claims should be directed to AXA Assurances Inc.
  5. Are industry employers who provide learning experiences for students on co-op work experience considered registered volunteers under volunteer accident insurance? No.
